Where did my Online Registration form go?

Here is the fix

1. Click "Settings" in the side bar menu.

2. Click "Online Registration" in the expanded side bar menu.

3. Change the "Registration Expires Date" to a date in the future

Click the "Save" button at the bottom of the page.
